Besides specializing in identifying needs and making recommendations related to those needs, Golden Care Partners is your professional resource for eldercare and senior services information and care planning supports. We can also provide the same individualized services to those who may be disabled or have ongoing significant healthcare issues.
Following a phone or email consultation, a recommendation of how we can be of service to you and your loved ones will be made. A comprehensive assessment, completed by a professional and licensed social worker, with individually tailored recommendations, is typically the first step. From there, we can assist as much or as little as necessary with planning, problem-solving, education, and advocacy activities.
Eliza ”Beth” Boltz is a Licensed Social Worker (LSW) in the state of Ohio. She is a dual degree graduate of Bowling Green State University, with a bachelor’s of science in Social Work and a master’s degree in Organization Development. Beth spent 16 years in public service on behalf of Wood County, Ohio, coordinating and monitoring services for citizens with developmental disabilities and their families. She has spent the last three years providing care management and short and long term planning services to Northwest Ohio area seniors and their families.
Beth is passionate about assisting individuals and their families with problem solving and identifying and meeting needs while fostering independence and creating an environment for growth. Owning and building Golden Care Partners has allowed this passion to become reality. When not working, Beth can be found spending time with or traveling with her family and friends, enjoying her grandchildren or swimming and gardening in her back yard. A native of Cleveland Ohio, she has made Bowling Green, Ohio her home for the last 30 years.
